CHM Referrals

Inclusion Criteria

• Confirmed COVID-19 positive (PUI excluded)

• Must have compromised respiratory status or respiratory systems directly related to COVID-19 (ex. SOB, O2 sats <94% on room air)

• Age 18+ (no pediatric patients)

• Not Pregnant

• KP South Bay Member or South Bay PCP (patient agrees to receive care at South Bay)

• Cognitively intact and able to self-monitor pulse oximeter and thermometer

• Tech savvy with smart device and Spanish/English literacy (or have family members in same household who can serve as communication surrogate)

• CANNOT be on Hospice or Palliative Care

Daytime Enrollment (8:30 am - 4:30 pm, 7 days a week)

• Cortext CHM doctor on call

• Schedule changes monthly, see below for current December schedule

Evening Enrollment (4:30 pm - 8:30 am, 7 days a week)

• Send referral message to CHM pool at P19581 and include: Name, MRN, date of discharge

• Check with Case Manager about getting patient a kit from ED

• If no kit is available, inform patient that CHM enrollers will schedule next weekday visit with Normandie North UCC PACE clinic RN for patient teaching and kit distribution
